Dr. Carolyn Gosse, B. Sc. Phm., ACPR, PharmD
VP Integrated Care & President St. Joseph's Home Care
Dr. Gosse is an internationally renowned leader in the development and adoption of models of Integrated Care. Since 2012, she has led the Integrated Comprehensive Care program across St. Joseph’s Health System (SJHS) and held the lead coaching role for MoHLTC Integrated Funding Models initiative.
Dr. Gosse is a LEAN six-sigma certified, business and clinical change management expert, with a series of research publication credits to date. She is a trusted advisor to the Ministry of Health, healthcare delivery organizations and sits on numerous provincial advisory panels and change initiatives.
Previously, Dr. Gosse was the Director of Clinical Programs at St. Joseph’s Healthcare Hamilton (Emergency Department, General Internal Medicine, and Urgent Care). Prior, she was the Director of Pharmacy Services having completed her Doctor of Pharmacy degree at the University of Florida in 2005.
Fraser Edward @FRE_Health
VP Partnerships & Business Development
Mr. Edward is a seasoned healthcare executive who works with public and private sector healthcare stakeholders, guiding them in the adoption of new and innovative care delivery models. With 20+ years of experience in the Tech Industry, he is passionate about the important impact that new models of Integrated Care, Artificial Intelligence and Information Technologies can have on transforming healthcare for all Canadians.
Mr. Edward serves as VP, Partnerships and Business Development at St. Joseph’s Health System and on the Board of Directors at Guelph General Hospital. Previously he has held Marketing and Business Development leadership roles at Research In Motion (BlackBerry), TELUS Health, Hewlett Packard and various start-up / growth stage Healthcare technology providers.
Dr. Andrew Costa, PhD @Andrew_P_Costa
Dr. Costa is an Assistant Professor and Schlegel Research Chair in Clinical Epidemiology & Aging in the Department of Clinical Epidemiology & Biostatistics at McMaster University. He also serves as the Research Lead at the DeGroote School of Medicine, Waterloo Regional Campus.
Dr. Costa is an Adjunct Scientist with the Institute for Clinical Evaluative Sciences (ICES) and is Lead Scientist, Big Data & Geriatric Models of Care Research Cluster, in the department of Health Research Methods, Evidence and Impact at McMaster University.
Dr. Costa is an interRAI Fellow where he is engaged in the Network of Excellence in Acute Care (iNEAC) and leads the Emergency Department Working Group. He has received CIHR awards for his research in health services and policy.
Carrie Anne Beltzner @CABeltzner
Coach and ICC Program Lead
Ms. Beltzner leads the development and delivery of educational and coaching services at St. Joseph’s Health System’s Centre for Integrated Care. Ms. Beltzner directs a team focused on enabling the adoption and implementation of the Integrated Comprehensive Care (ICC) evidence-based Model of Integrated Care for healthcare service providers, including University Health Network, Toronto. Ms. Beltzner also maintains oversight of a regional Integrated Comprehensive Care program for Complex Chronic patients with COPD and CHF, across 9 Hospitals across the Ontario Health – West Region.
Ms. Beltzner is a strong advocate for the Addictions and Mental Health sector, serving as Chairperson for the Peel Addiction Assessment & Referral Centre (PAARC) a non-profit community-based addictions agency.

The Centre for Integrated Care, draws upon clinical, financial, operational and academic subject matter expertise from our ‘Integrated Care – living lab’ across St. Joseph’s Health Systems members and partners.

Strategic Advisory Committee
The Centre for Integrated Care benefits from the expertise of a Strategic Advisory Committee co-chaired by Melissa Farrell, President SJHH and David Wormald, President SJHCG. Meeting quarterly to provide advice and direction, this diverse group of patients, physicians, clinicians, scientists and health leaders ensure the Centre achieves its mandate.
