St. Joseph’s Health System (SJHS) is an internationally recognized leader in the development and implementation of new Models of Integrated Care. Our perspective is founded in our health system’s service delivery capabilities across the complete continuum of care; coupled with our commitment to academic research and discovery. Our extensive experience in the adoption of Models of Integrated Care provides a unique vantage point from which to share our learnings and partner to further the field of Integrated Care.
The SJHS Centre for Integrated Care is a ‘practical hands-on’ resource for our System members and strategic partners to help drive the development, adoption and evaluation of models of Integrated Care; with additional research, educational, advisory, quality improvement, performance management and partnership activity.

In 2012, St. Joseph’s Health System pioneered an approach to Integrated Care for Surgical and Chronic Complex patient populations. Our model proved very successful and the Ministry of Health asked us to coach other organizations across the province. We called our model: Integrated Comprehensive Care (ICC).
What is Integrated Comprehensive Care?
ICC is an evidence-based Model of Care that supports patients with One Team, One Record, One Number to Call, 24/7. ICC is enabled by a co-designed integrated service delivery model and an integrated funding model, which we refer to as One Fund. Our focus is to make the patient experience as seamless as possible, while utilizing existing resources across the healthcare continuum, to deliver an integrated and comprehensive care experience. ICC enables sustainable transformational system change.
Patient and Provider Co-design
The principle of patient and provider co-design and the iterative nature of program development support the creation of trust, transparency and common purpose for all those involved in delivering care across the patient’s continuum of care. ICC as a program is adaptable over time, which ensures its ongoing effectiveness. ICC’s evidence-based approach must be firmly embedded in the cross-organizational culture, both in the clinical and administrative functions.
Our ICC program has resulted in a capacity saving of up to $4,000 capacity savings per patient, realized through a 30% reduction in emergency department visits and 30% reduction in hospital re-admissions; with a 98% patient satisfaction approval rating.
